Sleep Terrors

Sleep terrors are commonly known by a few terms, including sleep terrors, night terrors, sleep terror disorder, and pavor nocturnus. The person having a sleep terror will awaken from a dream crying or screaming, which may last for minutes. The person is difficult to awaken and confused. There is no recollection of the dream once awake. Sometimes the person can be violent by swinging their fists or punching. This paper will look in depth at sleep terrors and give a concise review of the causes, prevalence, treatment, symptoms, and safety measures in the sleep lab when dealing with sleep terrors. Definition The definition of sleep terrors is â€Å"a sleep disorder involving abrupt awakening from sleep in a terrified state† (Kiriakopoulos, 2005, p. 1). â€Å"The cause is unknown but night terrors are commonly associated with periods of emotional tension, stress, or conflict† (Kiriakopoulos, 2005). Overview To understand sleep terrors, you first need to understand normal sleep patterns. â€Å"Normal sleep cycles involve distinct stages from light drowsiness to deep sleep† (Kiriakopoulos, 2005, p. 1). These include REM and non-REM sleep. Non-REM sleep involves four stages. The normal sleeping person will have several cycles of REM and non-REM sleep through the night. Night terrors occur during deep sleep, which occurs during stages 3 and 4 of non-REM sleep, beginning approximately 90 minutes after falling asleep (Connelly, 2003) and they generally have a duration of 10 to 20 minutes. The patient usually screams in panic, but there is no memory of what caused them to be scared. (Kiriakopoulos, 2005) Sleep terrors are like nightmares but with certain distinct differences. Nightmares normally occur during REM sleep vs. sleep terrors occurring during deep sleep. Nightmares are most common in the early morning vs. sleep terrors occurring during the first one-third of the night. The sleeper can remember the details of a nightmare vs. a sleep terror, which cannot be remembered. Sleep terrors are not normal, but â€Å"nightmares are normal on occasion, especially after frightening movies/TV shows or emotional situations† (Kiriakopoulos, 2005, p. 1). â€Å"The sleep disorder of night terrors typically occurs in children aged 3-12 years, with a peak onset in children aged 3? years† (Connelly, 2005, p. 1). Boys, girls, and children of all races are equally affected by sleep terrors. An estimated 1-6% of children experience night terrors† (Connelly, 2005,p. 1). Sleep terrors can be familiar. By adolescence, most children have outgrown sleep terrors. (Connelly, 2005) Sleep terrors can run in families. Although uncommon, adults may have sleep terrors but they are associated â€Å"with emotional tension and/or the use of alcohol† (Kiriakopoulos, 2005, p. 1). Although sleep terrors are â€Å"not directly caused by a general medical condition or substance use, including medications and drugs of abuse, these symptoms cause clinically important distress or impair work, social or personal functioning† (Narcolepsy Association UK, 2003). Diagnosis â€Å"Usually, a complete history and a physical examination are sufficient to diagnose night terrors† (Connolly, 2003, p. 6). Review of symptoms is necessary for accurate diagnosis of all sleep disorders. In many cases, considering the symptoms, no tests are necessary. (Kiriakopoulos, 2005) It is important to rule out seizure disorders and nightmares when making the differential diagnosis of sleep terror. (Narcolepsy Association UK, 2003) This may include an electroencephalogram (EEG) which measures brain activity and can rule out a seizure disorder, or a sleep study to rule out other sleep disorders. Other tests such as CT Scans and MRI’s are unnecessary. (Connelly, 2003) The cause is unknown. Sleep terrors are frequently brought on by â€Å"stress, fever, sleep deprivation, and medications that affect the central nervous system† (Connelly, 2003, p. 2). Symptoms Symptoms of sleep terrors include sudden awakening from sleep. â€Å"During each episode the patient shows evidence of marked fear and autonomic arousal, such as rapid breathing, rapid heartbeat and sweating† (Narcolepsy Association UK, 2003). When awakened from the sleep terror, the person is difficult to comfort and is unable to explain what happened or recall the dream, though they may have a sense of being frightened. â€Å"They will have no memory of the event on awakening the next day† Kiriakopoulos, 2005, p. 1). Thankfully, sleep terrors generally only occur once per night. (Narcolepsy Association UK, 2003) Treatment, Prognosis, And Prevention There is no true treatment for sleep terrors. It is important to â€Å"educate the family about the disorder and reassuring them that the episodes are not harmful† (Connelly, 2003, p. ). â€Å"Comfort and reassurance are the only treatment required† (Kiriakopoulos, 2005, p. 1). It is important for parents of children with sleep disorders to take the following precautions. â€Å"Make the child’s room safe to prevent the child from being injured during an episode, eliminate all sources of sleep disturbance, and maintaining a consistent bedtime routin e and wake-up time† (Connelly, 2003, p. 7). When medication is used, it is benzodiazepine or benadryl given just prior to or at bedtime. These medications will reduce sleep terrors. Although rare, â€Å"in severe cases, in which daily activities (for example, school performance or peer or family relations) are affected† (Connelly, 2003, p. 8), the person may require antidepressant therapy. â€Å"Minimizing stress or using coping mechanisms may reduce night terrors. The number of episodes usually decreases after age 10† (Sheth, 2005, p. 1). Most children outgrow night terrors. When this does not occur, â€Å"stress reduction and/or psychotherapy may be helpful for sleep terror in adults† (Kiriakopoulos, 2005, p. 1). â€Å"Night terrors may also be treated with hypnosis and guided imagery echniques† (Narcolepsy Association UK, 2003, p. 1). There are also support groups available. Role of the Polysomnographic Technologist The sleep technologist is responsible for the patient during the polysomnography study. The technologist needs to have reviewed the patient’s questionnaire and be prepared for the outburst and confusion that may occur during a sleep terror event. The technologist needs skills to comfort and protect the patient during a terror. The patient may be combative so the technologist would need to be able to protect the patient and any near him from hitting and punching that could occur. It would be important to be able to gently restrain them so they are both comforted and safe. Polysomnographic Record Of A 3 Year Old With Sleep Terror Disorder Summary/Conclusion Sleep terror disorder is a sleep disorder that involves waking from deep sleep in a panic. The person is difficult to arouse and confused. If they can recall any of the dream at that time, when they wake then next day, they will not remember that the terror even occurred. The disorder occurs mostly in young children before their teen years begin. Most children out grow the disorder, but some adults are affected. Diagnosis is simple, requiring few tests. Treatment is mostly supportive requiring education of the family and comfort of the individual with the sleep terror disorder. In rare cases, psychotherapy and/or counseling may be necessary if it affect the persons daily functioning and relationships. Sleep terrors are very interesting to me. I have never even known anyone with terrors, and but the prevalence is enough that most people should know someone with the disorder. Although the diagnosis is usually easy, the sleep technologist has a role in the diagnosis when more than the patient’s symptoms are needed.

Tresidder (2005) gives some examples like, the individual standing stones of specific shapes being worshipped, sacrificial stone axes or knives in the Neolithic age, and using stones to mark sacred places or act like altars etc. In funeral ceremony stones were used, as also by certain religion in the modern world for symbolizing eternal life. In addition to the above examples Tresidder (2005) also mentions that the stones having specific shapes, sizes and colors were also used as amulets. Many people used to believe that these stones bring the wearer good luck, while others used them to ward off certain mishaps. Many theorists believe that this was the origin of birthstones as we know today. Gemstones Schumann (1993) says that gemstones have been known to men for more than 7000 years. The earliest gemstones belonging to the precious category were the amethyst, garnet, amber, jade, emerald, turquoise, lapis lazuli etc. Possession of these stones was considered to be a status symbol among the royal and other wealthy people. Hence, these stones came to be associated with power in addition to being used as amulets and talismans. These stones depending on their history were used to sustain health or bring seafaring people safely and mainly to ward off evil. As time passed on, special stones started to have specific meaning based on various myths, legends and symbolic meaning attached to them. (Dunwich, 2003). In modern times this number has increased multifold. The semi-precious stones were the stones which were less valuable and not very hard. These stones even today are used to denote the stones which are of lesser value than the precious stones, though the difference between them is not very clear. In present times the quality of stone and at times even their occurrence i. e. natural or synthetic, decides whether a stone is of the precious or semiprecious type. Gemstones associated with Zodiac The association of gemstone with the zodiac was not just associated with a single race or religion in ancient times. Many cultures had similar beliefs. Walker (2002) says that, since ancient times Christians and Jews related certain stones as to the twelve tribes or Israel and to the twelve apostles. Ancient Jewish cabalists believed that the stones each one having an anagrm of the name of the God had mystical power over the twelve angels. The above Jewish practice can be traced back to biblical times, specifically to the breastplate of Aaron (Dunwich, 2003), who was a Hebrew High Priest and a brother of Moses. This breastplate is known as the Breastplate of High Priest and is said to be the oldest amulets known to man. Dunwich (2003) says that according to Bible this breastplate was made of the following gemstones: sardius, topaz, carbuncle, emerald, sapphire, diamond, ligure, agate, amethyst, beryl, onyx, and jasper. These twelve stones, as mentioned earlier used to represent the twelve tribes of Israel, the twelve months of the year and the twelve signs of the zodiac. Gleadow (2001) says that the most common list of birthstones can be derived from the Book of Revelation, beginning with St. Peter, Aries and the month of March and assuming the Jasper of that month to be red. However, there were many changes made to this list and the list available by different religions or different countries differ a lot and tend to disagree. Modern civilization can trace its routes quite clearly to ancient history, one civilization after the other, with each influencing the next and all subsequent empires, in terms of both innovation and invention. Perhaps one of the most advanced empires of its time, in terms of technological development and innovation, was the Persian Empire, which occupies the area now held by modern Iran. This was perhaps due to the fact that their own technological innovations and ideas were greatly helped in development due to the natural resources available in the area. The Persian empire can not only be attributed to inventions such as the Qanat, a water irrigation system, windmills, or even the earliest batteries – which were possibly used for purposes such as electroplating – but also to a great number of technological advances, in terms of weapons as well as art and architecture. The Persians had managed to develop their ancient tools of knives and bows and arrows into stronger, more advanced weapons made of hard metal by the rule of Darius. This included both weapons, such as the metallic dagger and swords, as well as armor, such as shields and helmets. Persian architecture included the use of cider instead of stone, allowing them to create tall, high structure, supported by thinner and more delicate, yet stronger, beams than other buildings of the era. The Persians can also be attributed to having invented history’s first pontoon bridge. Many of these inventions served as inspiration for future development and some of them are in fact still used today, but in a technologically more advanced form. As advanced as the Persians might have been in their own time however, there is absolutely no doubt as to incredibly advanced level of innovation and inventio n held by the Greeks. Inventor and mathematicians such as Heronas, who was the mind behind modern day hydraulic systems as well as many other machines and automation systems, was also attributed to having invented and developed the world’s first steam engine. The steam engine, in its developed modern form, was one of the most milestone inventions for modern western civilization as it provided a complete turn of mechanism from the ancient days of doing everything by hand. It created a clear divide between the age of man and the age of machines, with modern civilization developing rapidly and dramatically after its invention. Indeed many of our inventions today can trace its route back to the steam engine, or can at least accredit it to being the inspiration behind them. Ethnicity and Education Essay Most studies found that overall ethnic minorities tend to do less well than other members of the population when it comes to educational achievement. The government sponsored â€Å"Swann report† found that â€Å"Asians† did almost as well as â€Å"Whites† however not as well, one Asian group in particular the â€Å"Bangladeshi’s† did particularly badly and â€Å"West Indians† preformed considerably worse than â€Å"Whites†. Many factors which occur inside the school are the cause of educational achievement however other factors contribute. The expectations of teachers on ethnic minorities have caused differences in educational achievement. Bernard Coard believes that teachers expect â€Å"Black† children to fail therefore labelling the child as a failure. Interactionalists are concerned with how teachers â€Å"pidgeonhole† students this is the labelling theory, this is where the teacher makes a judgement of the child based on the child’s appearance, behaviour, previous family members and relationships with other students. However most labels are based on stereotypes rather than evidence, the reality is that once a label starts it is difficult to get rid of and this can cause problems for a student’s educational achievement. Teachers will discuss the students label in the staff room and other teachers will attach the same label to the child and this can lead to the child not being allowed to attend school trips. If a student is labelled as a failure by a teacher then the teacher is likely to put the student into a lower tier exam thus the pupil can only achieve a maximum of a grade C and can’t achieve to the best of their ability. Bernard Coard states that the British educational system makes children become educationally subnormal he states that â€Å"West Indian children are told that their language is second rate and unacceptable† he states that within the British educational system the word â€Å"White† is associated with â€Å"good† and the word â€Å"Black† with evil. Coard also states that the attitudes portrayed in the classroom are reinforced by the pupils outside of the classroom. Many sociologists would also argue that the mispronunciation of student’s names and not learning them can be classed as racism within the school system. Cecile Wright found that majority of staff are committed to ideals of equality but despite these ideals there are still considerable amounts of discrimination in the classroom. Asian children were often excluded from discussions and teachers expressed an â€Å"open disapproval of their customs and traditions†, these comments from teachers made Asian children feel more isolated from other pupils. These are pupils who have rejected some or all of the values and rules set down in the school ethos. Peter Woods identified 8 different groups ranging from Ingratiation total conformity to Rebellion outright rejection of school rules and values. The Ethocentric curriculum can cause ethnic differences in educational achievement this is the school being biased towards one particular culture. Many sociologists argue that the curriculum is geared towards white middle class students. The curriculum is set in place for white middle class students and this means that ethnic minorities will not understand the curriculum and will fall behind and not do well thus leading to educational underachievement. Parents from ethnic minorities may not be able to afford or have the right experience to make an informed choice on the best school to suit their children’s needs whereas middle class parents have the money and correct experience to make an informed decision on the best school for their child. Ethnic differences in educational achievement are the result of school factors however other factors have significance. Material Deprivation is the effect poverty has on educational achievement. Poorer parents from ethnic minorities may not be able to provide their children with the educational toys needed such as computers. Cramped housing means less space to study. High rate of illness can lead to missing days of school. Children from poorer backgrounds may need to contribute to the financial running of the home thus getting part time jobs which will affect their studies and will leave school at the earliest opportunity to start work. Harker states that overcrowding causes a negative effect on educational achievement which can cause differences in ethnic minorities which can cause bullying and truancy. Cultural Deprivation is the term used to describe serious inadequacies in the socialisation process and its effects on educational achievement. Poor parenting means children underachieving in education. Many sociologists argue that ethnic minorities tend to be more fatalistic meaning that parents tend to have low aspirations and expectations for their child meaning less emphasis is placed on doing well in education. Whereas â€Å"White† middle classes would have high aspirations and high expectations. Socialisation focuses on the values, expectations and norms transmitted from parents to children. Children from Ethnic tend to focus on immediate gratification where they try to reep their rewards at the earliest opportunity such as leaving school at 16 to earn money. Whereas â€Å"White† middle class children aim for a bright future described as deferred gratification which is putting off short term rewards to gain better rewards in the future. Cultural Deprivation can affect the way parents prepare their children for school. Working class children would sit and watch T. V. whereas middle class parents would take their children to museums. Driver and Ballard state that high achievement in Asian groups may be linked to the fact that they live in close knit families that can help each other. Linguistic Deprivation is the ability to use appropriate language which is an important factor in educational success. Bernstein came up with two codes of language Restricted and Elaborate codes. The Restricted code is used in casual situations. This is limited vocabulary and Simplistic grammar which can only be understood if you are aware of this topic. The Elaborate code is used in formal situations. This is sophisticated grammar and vocabulary and can be understood if aware of the topic. Bernstein found that White middle class children had the ability to switch between the two codes whereas children from ethnic minorities were limited to using the restricted code. Germany was separated into 4 sections controlled by Great Britain, France, the Soviet Union and the United States and the Cold war started because there was a disagreement on unifying Germany. There were events that happened during the cold war the first was the foreign aid policies, which were able to divide the superpowers after that treaty organizations and alliances started forming up again one of these alliances was the North Atlantic Treaty Organization (NATO), as well as the Warsaw pact.
